MOS filing

From 27.04.2026 temporary stay, permanent stay and EU long-term resident applications are as a rule MOS-only (mos.cudzoziemcy.gov.pl). Paper that did not reach the voivode before that date is left without consideration. Paper still applies on the UDSC list: intra-corporate transfer (ICT) and ICT long-term mobility; family reunification / PL–EU–UK family member / ECHR family life when the foreigner is outside Poland.

Who signs

The foreigner (or statutory representative / parent) fills the form on their own MOS account via login.gov.pl and signs with a trusted profile or qualified e-signature (UDSC also lists a personal signature). A proxy cannot open an account, share logins, or send from MOS in your place. Help is allowed while you fill the form in your presence. The decision goes to the delivery address you name (yours or a proxy’s).

In person

Electronic filing needs no appointment. The voivode will summon you in person for fingerprints, a specimen signature and the original passport, and may ask for extra documents.

Fees / annexes

Attach in MOS: a digital photo, scans/photos of all passport pages, and payment proof. Fees are not paid inside MOS (UDSC: stamp duty 340–640 zł by permit type + residence-card fee 100 zł — state fees, not the ROUTEPOL price). Work / EU Blue Card / company-function / studies / internship annexes are e-signed by the other party.

UPO / status

Keep the UPO. After office confirmation, a filing certificate (replacing the passport stamp) is downloaded from MOS; stay may remain lawful until a final decision. Wezwanie / notices are summarised. Some voivodeships send an inPOL code — logins are not shared.

Decision

The voivode decides; service is to the address you name (yours or a proxy’s). Appeal follows the deadline and Head of the Office for Foreigners procedure stated in the decision. Card / permit outcome is not sold.
